New Patients: From the first day you join our office, we will cater to your dental needs. As part of your first visit with us, you will fill out a comprehensive medical and dental history. These forms are important because they help us to determine the course of your treatment. We have to know about allergies, sensitivity to anesthetics, long-term medications, medical conditions, etc.
For your convenience we have our new patient forms online. Please print and read the HIPAA notice (below) before filling out the new patient forms.
Insurance and Payment
We don’t want to let expenses prevent you from benefiting from the quality of care you deserve. Our goal is to maximize your insurance benefits and make any remaining balance easily affordable.
We accept the following payment options: cash, personal checks, money orders, Visa, MasterCard, Discover, and American Express. We can also arrange a financial plan that best suits you.
Our practice is a provider for the Delta Dental Premier plan. Dr. Law is a United Concordia Alliance provider. As a courtesy to our patients, we will submit dental insurance claims on your behalf to most other insurance plans as well. You will be expected to pay your estimated out-of-pocket expenses at the time of treatment or establish a payment plan for that amount. We ask that you keep us updated with your current insurance information so we may accurately bill your insurance in a timely manner.
We are also pleased to accept CareCredit – No Interest* financing and Lending Club Patient Solutions no interest or low minimum monthly payment options so you can get what you need, when you need it.
